For patients who have never been certified in the Minnesota Medical Cannabis Program. Our licensed NP conducts a thorough telehealth evaluation and, if eligible, certifies you with the MN OCM.
For existing patients whose certification or enrollment has expired or is approaching expiration. Stay continuously enrolled in the Minnesota Medical Cannabis Program without interruption.
Your appointment request has been sent to Assured Health. Please complete your payment via Square to lock in your time slot. A confirmation email with your telehealth video link will follow.

Review your appointment summary and complete secure payment via Square. All major cards accepted.
Join your telehealth visit via the link we email you. If eligible, we submit your certification to the MN OCM.
